This Privacy Policy applies to precisiontotalhealthandwellness.com and to the personal information we collect from patients and visitors through this website. Precision Total Health & Wellness Clinic, LLC is a nurse-owned medical practice located at 101 E. Mahoney St, Suite 11, Plant City, FL 33563.

Precision is a direct, cash-pay practice. We do not bill health insurance. Because we do not file insurance claims, we are not a HIPAA covered entity. We still treat your personal and health-related information as confidential, use reasonable safeguards to protect it, and do not sell it. We also follow the Florida laws that protect personal and medical information, including the Florida Information Protection Act (Fla. Stat. 501.171) and Florida's medical-records confidentiality law (Fla. Stat. 456.057).

Payment card details are handled by our payment processor and are not stored on this site. No website or method of electronic storage is perfectly secure, so while we work hard to protect your information, we cannot guarantee absolute security.

How long we keep your information

We keep your information for as long as needed to provide care, run our practice, and meet our legal and recordkeeping obligations. Medical records are retained for the minimum periods required by Florida law (Fla. Stat. 456.057). When information is no longer needed, we take reasonable steps to dispose of it securely.

Your choices and rights

You may ask us to review or correct the personal information we hold about you, and you may request a copy of your medical records as allowed by Florida law. You can also ask us to stop sending non-essential communications at any time. To make a request, contact us using the details below. We may need to verify your identity before acting on a request.

Analytics

This site uses Google Analytics to understand how visitors use it. Google Analytics sets its own cookies in your browser and receives information about your visit, such as the pages you view, their full addresses (which can include a campaign or ad click code, the service you chose to book, or a reference number for a booking you have just made), the website that sent you, and basic details about your device and browser. It also receives a note when you tap one of our phone numbers or successfully send a contact or booking request, and, when you send the contact form, the answer you chose for "How did you hear about us?". It never receives your name, email address, phone number, or the message you write.

Advertising

This site uses no advertising cookies or ad pixels. It runs no Google Ads code.

We do advertise on Google. We measure how those ads perform with Google Analytics: when you arrive from one of our ads, the link carries a campaign code, which Google Analytics receives as part of the page address, as described above. If you call us using the call button in one of our Google ads, Google counts that call itself, as part of the ad, not through this site.

Cookies and your choices

When you first visit, a banner asks whether you accept cookies. Google Analytics cookies are on unless you choose Decline.
