The Visit
One appointment, one price, and the form goes home with you
Every school, league and program in Hillsborough and Polk hands out its own version of the same paperwork, and the exam behind it is the same one. So we do not sell four different physicals at four different prices. Your athlete gets a history and a hands-on examination with a nurse practitioner, you get the findings explained to you rather than handed to you, and the form is dealt with while you are still in the room.
Bring the form your school, league, or program gave you, and when your physical clears you, we complete and sign it the same visit, so you leave with it in hand.
Visits are by appointment, which is how we keep a family from sitting in a waiting room during back-to-school week.

Before you book
How much is a sports physical in Plant City?
A sports physical is $35, flat. The school physical, the college physical and the camp or program physical are the same exam at the same price, so there is one price and one appointment to book. Nursing and clinical program physicals are $35 as well.
Will you fill out and sign my school's form?
Yes. Bring the form your school, league, or program gave you, and when your physical clears you, we complete and sign it the same visit, so you leave with it in hand.
Do I need an appointment for a sports physical?
Yes. Precision Total Health is appointment only, so book online or call ahead and we will reserve your time. We are not able to see patients without an appointment, and booking in advance means your paperwork is ready before you arrive.
Do you have Saturday appointments?
Yes. We hold Saturday appointments from 10 AM to 2 PM, which is usually the easiest slot for a family during the school year.
Who performs the exam?
Your exam is performed by a nurse practitioner. Precision is a nurse-owned practice, and the same provider who examines your athlete is the one who talks you through the findings.
What should we bring?
Bring the form your school, league, or program gave you, and a list of any medications and past injuries. If your athlete wears glasses or contacts, bring them, and wear clothing that is easy to move in.
